Transition from Primary school to Secondary school – SEND students

At Life Lessons, we recognise the importance of additional support for our most vulnerable students transitioning out of Primary School.

We have developed a child-led intervention, supporting them to consider their emotional, physical and educational needs and examine their strengths and skills. This will help them to feel confident with the increased independence and changes in Secondary Schools.

There are five cycles of intervention, which can be delivered flexibly to suit your context and each young person’s needs:

  • Cycle 1: Exploring emotions – understanding the powerful emotions we might have around transition, and how to manage them.
  • Cycle 2: Exploring skills – understanding the skills that we have that will benefit us during transition.
  • Cycle 3: Exploring strengths – understanding the qualities we have that make us unique and important.
  • Cycle 4: Exploring needs – understanding our own needs and how to communicate these at secondary school.
  • Cycle 5: Exploring next steps – understanding how we can prepare for secondary school.

The resource utilises our popular narrative, discussion and reflection elements, as well as providing space for parents and other school adults to be part of the conversation. Implementation guidance notes are included. Through the 5 cycles of intervention, the child will build a ‘toolkit’, that they can use as a transition booklet.
